Consent for Collection of Personal Data

The parent/guardian and the applicant hereby consent that:

i. The Social Welfare Department (SWD) can use their personal data for the purpose of processing the application for “Strive and Rise Programme” (the Programme), and if the application is accepted, to join the Alumni Club in the future. On a need-to-know basis, the personal data may be transferred and disclosed to the concerned bureau(x)/department(s) or any engaged consultant/agency for the operation, monitoring and evaluation of the Programme, etc. as and when necessary.

The provision of personal data to SWD is voluntary.  The applicant has the right to request access to and correction of his/her personal data.  Such request can be sent to Senior Social Work Officer (Youth)2 at Room 942, 9/F, Wu Chung House, 213 Queen's Road East, Wan Chai, Hong Kong.  If the applicant does not provide the above consent, SWD may not be able to process the application.

ii. SWD can retain their personal data for the purpose of the next cohort of the Programme in case the applicant is eligible but is not admitted to this cohort.

If the applicant is not accepted for the next cohort of the Programme, his/her personal data will be destroyed within three months upon the commencement of the next cohort of the Programme.  An applicant may apply afresh for subsequent cohorts.
